Occasionally people have life events, such as illness or injury, which may lead them to need additional support for a short period of time, to assist with recovery or adjust to new challenges.
Short periods, of up to 6 weeks of re-ablement support, can prevent hospital admission or the need to stay in residential care, following discharge from hospital.
Cambridgeshire Homecare believes that it is important to focus on maximising independence through relearning skills that build self-confidence, which in turn, allows people to remain living in their own home for longer.
Re-ablement can support people to be less reliant on formal, traditional types of service in the longer term. The appropriate level of intervention is provided, with active and ongoing assessment, in order to reduce, or find a suitable level of support for the individual.
